New Patient Center

Welcome to HealthFirst Physical Rehab.  Conveniently located across from the North Towne Mall next to Burger King. 

On your first visit you will be greeted by a warm welcome and a smile by our front desk person.  Please make sure you bring a photo ID and your insurance information.  You will be asked to sign in, sign a few consent forms, and fill out a few forms on your personal history and information.  If you would like to save time you can print off the forms from the website, fill them out and bring them in with you.  Once you have finished filling out all of the information you will taken back to see the Dr.  He will perform an evaluation on you and decide whether x-rays will be taken.
